Admissions Procedure
At Careline Lifestyles, we adhere to a process of assessment that ensures the services we provide are specifically tailored to each individual service users needs and preferences.
We will always endeavour to ensure that our costs are based on the level of services and support that each individual service user requires. In order for us to do this, we undertake and deliver a full assessment report in partnership with a clinical assessment team consisting of Physiotherapists, Occupational Therapists and Clinical Neurological Psychologists.
Careline Lifestyles Application criteria and process is:
- Application - (Usually by a referral from Social Worker / NHS Primary Care Trust / Local Authority or Family Member)
- Applicants will usually be aged between 18 and 65 years old, have Physical / Learning / Neurological disability / Sensory impairment / Cognitive problems / acquired brain injury with enduring support needs
- All applications must be accompanied by a comprehensive assessment of need stating the reason for referral of application and the specific services sought (eg: short or long term / respite care etc)
- A pre-admission assessment will be carried out by Careline Lifestyles own multi-disciplinary team based upon observation and report
- Each application is given careful consideration by Careline Lifestyles team
- Admissions meeting held Everyone involved in the application including the service user, social worker, carers, representatives are invited to attend.
- Initial action plan drawn up identifying next steps once agreement has been reached to place the service user with Careline Lifestyles
- Trial - The initial three months of any new placement is considered to be a trial, allowing a period of adjustment and continued assessment, to establish if Careline Lifestyles can meet the service user’s needs and aspirations
- Approval based on client and service purchaser agreement
- Admissions and rehabilitative pathways
